Athletes cannot practice if the following school paperwork is not complete. Please take a moment and schedule an appointment with your physician. Please turn paperwork into the Athletic Office or to the High School or Middle School front desk.
Student Accident Insurance
PCA purchases supplemental insurance coverage for student accidents. This policy is not intended to replace individual, family, or group healthcare insurance coverage. Participation in athletic events requires the family subscribe to healthcare insurance coverage.
The student policy is accident only coverage and is not applicable to illness or sickness. The policy is limited benefit and may not cover all injuries or medical bills. Any bills not paid by the supplemental policy are the responsibility of the parent or guardian. Claims need to be filed within 90 days of the date of the injury.
